North Shore Family Health Team
The North Shore Family Health Team services the communities from Jackfish to Pays Plat First Nation. The team consists of a Nurse Practitioner, Registered Nurse, Dietitian, Social Worker and Administration Reception Staff.
The NSFHT (North Shore Family Health Team) is a provider-led Family Health team. Dr. Hurst is the lead physician for the NSFHT and Doctors Zawada, McLeod and Thompson are directors of the NSFHT.
The NSFHT is operated out of two clinics: J.E. Stokes Medical Centre in Schreiber and Aguasabon Medical Clinic in Terrace Bay. The team works collaboratively with other local agencies to ensure that needs are met. The team developed a self-management tool called “Passport to Health," a medical journal for patients to keep all their pertinent medical information in.

Équipe de santé familiale de la rive nord (ESFRN). Le médecin principal est le docteur Hurst et les docteurs Zawada, McLeod et Thompson sont administrateurs de l’ESFRN.
L’ESFRN comprend deux cliniques, le centre médical J.E. Stokes Schreiber et la clinique médicale Aguasabon Terrace Bay. Elle collabore avec d’autres organismes locaux pour s’assurer de répondre aux besoins identifiés. Elle a aussi mis au point l’outil d’autogestion « PasseportSanté », un journal médical permettant aux patients de conserver tous leurs renseignements médicaux pertinents.
